A case of paclitaxel induced scleroderma

Abstract

|
A 53-year-old woman visited our hospital because of abdominal distension. She was diagnosed as advanced ovarian cancer with peritoneal metastasis. From May 2003, she received combination chemotherapy consisted of paclitaxel 135 mg/m2 and cisplatin 75 mg/m2 every 4 weeks. After a further 1 month, skin of upper extremity and upper trunk became sclerotic. Scleroderma like skin thickening extended to neck and lower trunk and accompanied by contraction of all finger joints. But lower extremities remained unaffected, relatively. A skin biopsy from the right forearm showed diffuse dense dermal fibrosis with entrapment of mildly atrophic adnexal structures. Therapy with paclitaxel was discontinued and the patient was followed several weeks for change in the fibrotic process of the skin. Within 4 weeks of discontinuation of paclitaxel, the cutaneous tissues became soften. This is the first case of a scleroderma like skin reaction related to paclitaxel in Korea.(Korean J Med 70:S373-S376, 2006)
